【要不要開刀、手術?】
最近在閱讀有關腰部疾病、下背痛的教科書,裡面提及了「是否要進行手術,你可以先考慮的要素」,當中的概念我很認同,也詢問了一些醫師、治療師朋友們的意見,因此整理了以下要點,給在猶豫是否要進行骨科手術的人參考,這邊是以腰椎受傷為例,但共同原則可以應用到其他部位的骨科手術:

1. 除了骨科醫師,諮詢其他擅長非手術或保守治療的專業人員,例如復健科醫師、物理治療師等
每個專科例如心臟外科、骨科都需要數年來養成,優點是能專攻在自己擅長的領域,缺點是容易只用自己的角度、領域去處理問題。因此,同樣的一個問題,骨科醫師可能用手術、物理治療師可能用運動或徒手治療、復健科醫師可能用注射,重點是找到效果最好、副作用最少的治療方式。

2. 除非緊急或嚴重狀況,先進行高品質保守治療3-6個月,若未好再考慮手術
要決定進行一個醫療處置或治療,首要考量「好處是否大於壞處」,就像進行CPR,可能有壓斷肋骨的風險,但是有機會救活病患,否則就是死亡。因此,若能透過保守治療例如運動治療、徒手治療,甚至較簡易的注射等,即能達到跟手術同樣的療效,建議先進行保守治療,因為手術相對來說,仍是一個較大的傷害

3. 如果是「創傷、外傷」(trauma)或有顯著神經症狀(substantial neurological issues),可以考慮手術
外力或創傷例如車禍等,可能造成腰椎構造的嚴重不穩定,即可能需透過手術才能維持腰椎結構。顯著神經症狀指的是喪失排便尿功能、尿失禁(loss of bowel and bladder control)等,而常見的神經症狀包括轉移痛(radiating pain)、周邊麻木(peripheral numbness)、肌肉萎縮(muscle atrophy)等,初期可先嘗試保守治療或神經鬆動術等方式

4. 如果有嚴重疼痛持續超過三週以上,並且完全沒有改變
這裡指的是即做過許多治療後,疼痛程度完全沒有改變(unrelenting),並且是非常嚴重的疼痛(severe pain)

5. 跟手術的醫師有詳盡的討論包括:疼痛的來源、成功率、後續的復健等
許多情況下,影像所見到的問題不見得跟疼痛有關聯,意思是說核磁共振上顯示椎間盤突出,但疼痛有可能不是椎間盤造成的,因此詳盡地跟醫師討論並確認疼痛的來源很重要,例如透過動作檢測、理學檢查等釐清,並非單一只用影像學作為依據。
成功率除了指有沒有將骨頭固定好、穩定,還要詢問術後是否能回到「日常生活功能、原先的生活」,因為許多人預期手術後就一勞永逸,恢復到原先的百分之百,但實際上,儘管手術成功但在未做運動復健的情況下,可能無法恢復到原先能力的100%,因此務必與主刀醫師有完整的討論。

最後,手術屬於骨科醫師、外科醫師的專業,因此仍應由主刀醫師的建議及想法為主,因為沒有其他專業比他們更了解手術。但專業之間的合作、從更多不同觀點去處理統一個問題,才能達到最好的療效,在現今有限的醫療資源雖然較難達成多個醫療專業人員共同合作,但希望能逐漸往這一步邁進。

About a week ago, I sat down contemplating on how important kindness is and how much I’ve been preaching about it. Plus, I started wondering if I’m doing enough?

Then, I realized how much I’ve limited kindness to just ‘humans’ and forgotten that animals although mindless, can still feel pain. I’ve seen videos of how chickens, cows, pigs, goats, buffaloes, etc scream and cry in pain when being slaughtered to death or injected with fattening drugs, but I have always been consciously ignorant about them and brushed the thoughts off my mind.

I tell people to love animals and I sit here feasting on their flesh. To those of you who know me, you know I LOVE my meat. But my love for kindness is radiating stronger than before and I am prepared to go all out. I want to love ALL animals rightly. I want to be KIND to all living and non-living fairly. And slowly, but surely, I will.

I will never impose any of my practice/beliefs on any of you, nor will I judge meat eaters. To each his/her own. I often share my life updates with all of you and I feel like this, has got to be one of the biggest steps forward for me. And I am happy. I hope you are too!

In case you are wondering about the lack of updates here or on the blog, don't worry... I'm still here. It's just that I had an operation 4 days ago.
It had been a painful 2 weeks - with pain at my lower back and the pain radiating to my thigh and shin. So I was confined to bed the entire time and didn't feel like doing anything. Turned out that I had slipped disc, and a protruding small bone was irritating one of the nerves. I had the option of waiting it out but 2 weeks of pain and being barely able to walk was enough for me, so off to the operation I went. It was a relatively straightforward one: keyhole surgery and lasted only 1 hour. By the next day, I was up on my feet and walking around normally. Hooray!
I'm already discharged and resting at home but I just want to give thanks to my family for everything during the past 2 weeks. My Dad for sending the monkies to school every morning, my Mom for taking care of them and the wifey, my in-laws for accompanying the wifey during my op, the monkies for putting up with my tantrums (because it is very painful!) and of course, my wifey who was with me every step of the way and made sure that I kept my spirits up even when I felt I was so useless when I couldn't do anything. Love ya all many many! ♥
