莫迫幼童離開乳房

我收到的一封電郵：

「我現在接受每天一克 prednisolone（按：中譯，常用類固醇的一種）的靜脈注射，我應該繼續餵哺母乳嗎？這對我九個月大的兒子安全嗎？醫生建議我應該在接受治療的五天及之後的一天停止餵母乳。」

這媽媽得到的建議是不正確的。她不應該中斷餵母乳。Prednisolone 進入乳汁的量是非常少的，比方說，這遠遠低於為九個月大的嬰兒治療哮喘時所需的類固醇。

然而，最大的問題是很多人－尤其是醫生－似乎忘記了，要強逼這個年紀的幼童離開乳房，很可能會導致他情緒低落、心慌意亂。我知道有些醫生會這樣建議：「 長牙了，就不應該繼續餵哺母乳」或是「如果孩子已經大到會開囗說想要母乳，這是不正常的」。你錯了！自從哺乳類動物在地球上出現，所有母親一直都是這樣做的，那時還未有醫生呢。但對也好錯也好，事情的真相是，這個年紀的嬰兒，以至較大的幼童，都很愛在媽媽乳房哺乳，也很依賴哺乳。如果剝奪了他們這個安慰，可導致他們哭鬧多個小時、甚至是多日。當建議母親不要餵母乳時，特別是類似這案例的情況，建議者應明白，一刀切離乳對母乳幼童來說甚為殘酷。

這問題不僅出現在藥物治療中，還有許多其他情況，例如牙醫會建議母親不要給幼童餵夜奶，或完全停止母乳餵哺。又例如在分居或離婚的案件中，法庭會給予父親過長的探視時間，令母乳幼童長時間離開母親。

在經濟富裕的社會中，我們已經遺忘了母乳幼童是怎樣的、已經遺忘了什麼是親密的餵哺關係，因此我們亦已忘記幼童被迫突然遠離乳房所帶來的嚴重困擾。醫護人員需要明瞭這點，而不是動不動就勸說母親停止餵哺母乳，並毫無後顧之憂。
