妊娠糖尿怎麼辦?
孕媽媽:「我有個很瘦的朋友最近懷孕驗出妊娠糖尿,我都快將做飲糖水測試,萬一有妊娠糖尿病怎麼辦?」
CheckCheckCin:不論肥瘦的孕婦都有機會患上妊娠糖尿,是指懷孕前沒有糖尿病,但在懷孕時出現高血糖現象;因胰臟未能提供足夠的胰島素分泌,影響了血糖的調節,懷孕期間過度增重是高危因素之一,而高齡產婦、家族有糖尿病史、無運動習慣的孕婦患上妊娠糖尿的機會亦較高。一旦患上妊娠糖尿,有可能令胎兒過大、增加畸胎、早產等風險,所以要乖乖戒口以及做適量運動控制血糖水平。中醫角度看糖尿病屬於消渴病,妊娠糖尿成因多為肺、脾、腎三臟虧虛以致陰虛內熱,治療以益氣養陰為主,可以適量食用沙參、玉竹、百合、麥冬、枸杞子等食材。僅記少食多餐、定時定量,同時控制澱粉質的吸收,多吃粗糧及高纖食物,少吃煎炸油膩食物及甜品,切勿胡亂進補。

Managing gestational diabetes
“A friend of mine is very slim, but recently, she was diagnosed with gestational diabetes. I’ll be undertaking the Glucose Tolerance Test soon. What should I do if I’m diagnosed with it too?”
CheckCheckCin: Whether you are plump or skinny, all pregnant women have an equal chance of developing gestational diabetes. Gestational diabetes refers to a condition in which the blood sugar levels of a person, who previously was not diabetic, become high during pregnancy.
This happens because the pancreas does not produce sufficient insulin for the body. Hence, it is unable to regulate the sugar levels effectively, and excessive weight gain can be dangerous during pregnancy. Older pregnant women, those with a family history of diabetes, and women who seldom exercise have higher chances of having gestational diabetes.
Once a pregnant mom develops this condition, her fetus might become larger, teratogenesis (congenital malformations in an embryo or fetus) might happen, and premature birth might occur. Hence, it is important to watch your diet and exercise regularly to control your blood sugar levels.
From Chinese medicine perspective, gestational diabetes is mainly caused by the deficiencies in the lungs, spleen, and kidneys. These deficiencies would then cause the deficiency of the yin and the accumulation of heat in the body.
To improve the body condition, we need to strengthen the qi and nourish the yin. Consume an appropriate amount of glehnia root, polygonatum odoratum, lily bulb, radix ophiopogonis, and wolfberry. Remember to eat more meals but in smaller quantities and regularly. Do control the intake of carbohydrates as well. Consume more coarse grains and foods that are high in fibers, cut down on fried and oily foods and desserts, and avoid taking nourishing products unnecessarily.

糖是什麼?
糖 sugar 是泛指各種可以食用、帶有甜味的晶體。
我們每天吃的食物中,即使是鹹食,也會有許多糖在裡面。
雖然全世界80%的食用糖是由甘蔗提煉出來的
但像我們平常吃的米飯,麵包等等
當進入我們的消化系統後
都會和蔗糖一樣
會分解成為葡萄糖(Glucose)和果糖(Fructose)
有些化學結構不同的物質 也帶有甜味
像是赤藻醣醇,阿斯巴甜,糖精,這些稱作代糖
但代糖不會歸類為糖類,
因為他並不會被人體分解吸收
你們知道,食用糖已經被證實和肥胖症有強烈的關聯
除肥胖症以外,也會造成第二型糖尿病、心血管疾病、加速老化 ,蛀牙等等
糖為何這麼多問題,
首先我們需要瞭解當糖進入身體後
會經過什麼樣的生物化學反應
大部分的人,每天都會攝取到碳水化合物
當碳水被胃酸消化以後,
會被送到小腸
進一步的分解成為葡萄糖和果糖
當血液中糖分提高時
這時候胰臟會分泌一種賀爾蒙叫做 - 胰島素
你可以把胰島素想像成:將細胞打開的一個鑰匙
它可以讓葡萄糖進入細胞中,直接被利用,
或被儲存當作能量,將來再使用
這段代謝的過程 對於我們追求肌肉量的人很重要
因為肌肉細胞 和身體其他細胞一樣
是需要養份,才能夠長大
由於葡萄糖是活細胞的能量來源,
所以大部分的葡萄糖會立刻被細胞利用
剩於的會進入肝臟
而到肝臟的葡萄糖
大約一半的會由肝臟代謝,儲存為肝糖
只有極少數的葡萄糖會被轉換為脂肪
那果糖呢,則是完全不一樣的故事
像市面上有帶糖的飲料,甚至超商賣的水果汁
其中都有添加大量的果糖
而果糖在人體代謝的過程,和酒精相似度極高
和酒精ㄧ樣,
果糖不是人體運作的必需品
也不是可以直接利用的單醣
影片一開始有提到
當血糖濃度提高時,胰臟會快速的分泌胰島素
讓葡萄糖可以進入細胞中 讓血糖濃度降低
再細胞忙著在接受養份時
這時候身體是暫時停止燃燒脂肪的
因為果糖無法被細胞直接吸收利用
所以當果糖到了肝臟
並不會轉換成為可被利用的肝醣
而是會經過代謝後 轉換成為脂肪 (三酸甘油酯)
這些部分的脂肪,會被儲存在肝臟裡
而長期下來呢,就會造成過多的脂肪堆積在肝臟裡
形成了『脂肪肝』的問題
科學家也相信脂肪肝會嚴重影響胰島素的運作
當人體血糖濃度過高,又沒有辦法藉由胰島素將血糖壓低
這時候就很可能造成,第二型糖尿病產生
當肝臟的脂肪過高,血液中的脂肪含量也會隨著提高
更會造成內臟脂肪含量增加 讓肚子變的越來越巨大
血液中的脂肪也會再動脈內壁上堆積,
逐漸讓動脈變狹窄,血液流通阻力增加
幸運點可能沒有事
嚴重一點可能會導致中風,心臟病等等
以上的代謝過程都是非常簡化後的版本

😲EATING CARBS WILL KILL YOU?🤔
💥There’s been lots of sensational headlines around a new study that was released this week 😱 So we thought we’d do a quick summary.
🔎For a more thorough breakdown we’d recommend the reviews by Dr Joanna McMillan & Nutrition Wonk (linked below)
⭐️WHAT WAS THE STUDY?⭐️
🔬The research (called the PURE study), looked at the diets of 135,335 people in 18 countries in Asia, Africa, Europe and America.
🍏Researchers looked at what people ate & followed them for ~7 years. They recorded who died (& why), as well as who had a cardiovascular event (like a heart attack/stroke).
❤️They then used their data on peoples diets to find out if there was any link between the % of macronutrients (carbs, fats, proteins) people ate & their risk of dying from heart disease/having a cardiovascular event.
⭐️WHAT DID THEY FIND?⭐️
🍚They found that the people who ate the most carbs (~77% of their diet) had a 28% higher risk of death (from all causes, but not from heart disease).
🥑They also found that the people who ate the most fat (~35% of their diets) had a 23% lower risk of death than people who ate the least fat (~10%).
🍖The type of fat people ate didn’t make a difference to the risk of heart disease or cardio events.
🍗 Low intakes of saturated fat were associated with a higher risk of stroke.
⭐️SOO CARBS ARE BAD AND FATS ARE GOOD?⭐️
✋️Not so fast! There’s a few things in this study which aren’t immediately obvious from the headlines:
1. It was an observational study - this means it can show links and patterns but not cause & effect.
2. The people who ate less than 60% of energy from carbohydrate weren’t at an increased risk of death, this association was only seen at VERY HIGH intakes of carbohydrates, not moderate intakes, as seen in the UK.
3. This study is done predominantly in low & middle income countries and the results may be clouded by poverty. For example, protein & fat in poor countries are usually more expensive than carbs. This means that people who are poorer are likely to eat a lot more carbs and not through choice. Poverty also usually means poorer access to/adequacy of healthcare and diets which don’t meet energy and nutrient requirements. It’s hard to tease this relationship out and so it could affect the results.
4. The high fat diet wasn’t really high fat. In fact the highest fat group ate ~35% fat, which is similar to current UK recommended amounts. The effects at higher intakes of fats weren’t included in the study.
5. This study only looked at different types of fat and not different types of carbohydrate, the predominant source of carbs in Asia being refined white rice, for example.
⭐️WHAT DOES THIS STUDY TELL US?⭐️
👍Overall, this study is an interesting addition to the body of research on fats and carbohydrates.
⚖️Its findings are in line with other research that has indicated that we shouldn’t be afraid of eating a moderate amount of fat (a la med diet), and that we shouldn’t be replacing fats with carbohydrates in our diets.
🤓However, unlike the headlines suggest, it’s not an indication that all carbs are “bad” for us, or a green light to eat bucket loads of fat. It simply supports the notion that diets with moderate amounts of good quality carbs and fats can be healthy.
