Gotta be honest, this is one of the most challenging cases I’ve done so far. Single anterior discoloured tooth is always hard to deal with, especially with a gummy smile. But I’m excited to share!🤗
To achieve this result:
Smile Design • Gingivectomy • In-office Dental Whitening & Internal Bleaching • Mock-Up • 7 unit Composite Veneers & 1 unit Ceramic Crown
Patient has excessive gingival display (“gummy smile”) with hyperactive upper lip elevator muscle. We’ve discussed the options of:
🦷Orthodontic TX with TADs (“bone screw”)
🦷Botox injection to help reduce the upper lip elevation upon smiling
🦷…or simply accepting the profile and patient just have to learn to smile a little less wide
Patient opt for no Ortho tx nor Botox, and only the treatments I’ve mentioned above. Fully understanding the limitations of the improvement we can bring her, she trained herself to smile to her ideal height and I actually think it looks good.😊Sometimes, no treatment is also a form of treatment, don’t you agree?
Composite veneers weren’t too hard to place. However, it was difficult to mask the discoloured tooth. Root canal treatment was done many years ago and we can actually see the root discolouration showing through the gingiva. (Watch my story to learn more) Even with internal bleaching, the improvement was limited, unless we carry out soft tissue surgery to increase the thickness of the 21 buccal gingiva to mask the root discolouration.
We managed to mask some of the crown discolouration on 21 with procelain-fused-zirconia crown (to match the crown to the rest of the tooth was difficult as well) and the help of a little opaquer, yet there is still room for improvement in my opinion.
Although challenging, I’m blessed to have a very kind patient who’s willing to go through the whole treatment with me. There’s no difficult treatments, as long as our partner (aka patient) is a great one. Of course, it is cases like this which help us become better clinicians. After getting her Ortho (braces) treatment completed, she wanted her “bunny teeth” to be corrected as well. 🐰 Now the question is, what makes her teeth “bunny-like”? Large Centrals? Small Laterals? Low gum line? Or maybe….all?🤓

Note that her 21 axis is a little slanted (in simple words, “Senget”🤣) and she want it corrected as well. Upon mock-up, we have discussed about the need for Enameloplasty to achieve the tooth form she wanted, or to increase veneer thickness (which wouldn’t look as nice as planned but we still offer it as a treatment option because…patient’s decision matters!😊). She decided to proceed with Enameloplasty so we went with it.
I will always try to keep my Enameloplasty minimal and within enamel, so the resin bonding would be more ideal.
It was really fun working on this case as patient knows what she wants and is super understanding as well. What makes our job great is most often not the set of teeth itself, but the patients we work with!😍

Fiber-reinforced Bridge…do you know what that is?🤔
When you lost a tooth or teeth, you have a few options to replace it: denture, bridge, implant, braces or more.
She first came in with loose denture replacing her two front teeth. Denture is a good replacement option for missing teeth but with time, it tend to become loosen due to bone resorption (“bone thinning”). Loose denture can be troublesome as it falls out easily (no one wants their teeth falling out during a presentation!😱) and food often get stuck underneath it.
If you ask me, implant is the best option to replace this patient’s missing teeth. However, implants (and not forgetting bone graft) placement can be costly and she is not financially ready for it. Having the option for a new denture or a fiber-reinforced bridge, she decided to choose the latter whilst preparing for her implant budget.
The challenge in this case is the total buildup of the tooth with composite, estimating it’s size/shape/shade and not forgetting occlusion. This is a non-prep case as the patient is biting rather incisally on the anteriors while having good posterior occlusion(thus made her a good candidate for this long span FRB case).
Most of it has to be done chair-side, thus a lot of clinical time has been spent. Note that the 11,21 are shortened as well although I would LOVE to give her a perfect smile arch, due to concerns of her anterior guidance.
Patient was super happy she has semi-permanent teeth inside her mouth and don’t have to worry about her denture falling out anymore. (We both still look forward to the day implants can be placed tho!🙈)
Case selection is important in FRB treatment. It is not for patients who want an easy way out, but for patients who clearly understand ALL the pros and cons of their treatment options and are willing to accept the worst-case scenario.
