【＃滿載而歸 10】因為愛，讓他好好走​
​
我一直都對生死議題很有興趣。​
​
那個靜默的陽光午後，是部記錄大體老師以及家屬​
在老師教學這段時間的種種思念。​
在看完這部片之後，我開始去思考死亡前的準備​
以及面對死亡這件事。​
​
我好奇要怎麼成為一位大體老師。​
成為一個大體老師，必須不能得過法定傳染病、​
不能有重大創傷、不能自殺、需滿16歲，​
動過器官摘除及捐贈手術，甚至太胖或太瘦也不行。​
​
也因此，查閱資料時，我第一次聽到DNR這個名詞。​
DNR，全名為Do-Not-Resuscitate(不施行心肺復甦術)：​
​
意思就是當病人罹患嚴重傷病，​
醫師診斷認為不可治癒，​
而且病程進展至不可避免的死亡程度時。​
病人或家屬同意在臨終或無生命徵象時，​
不施行心肺復甦術(包括氣管內插管、​
體外心臟按壓、急救藥物注射、心臟電擊、​
心臟人工調頻、人工呼吸或其他救治行為)。​
​
目前網路下載表格及各大醫院都可以申請，​
也可以將其註記在健保卡上。​
​
我也想申請，避免哪天辛苦的走完人生。​
我以為只要申請，​
就可以避免掉這些又痛又苦的急救措施。​
​
前陣子，我讀完重症醫學醫師黃軒的作品​
《因為愛，讓他好好走》，​
重新認識了DNR這件事。​
這本書，帶我改變了三個觀點：​
​
一、簽署了DNR，不代表我能善終。​
​
書中提到了許多已簽DNR，但家屬卻因為不願放手，​
希望醫師繼續急救，於是只好插管、CPR，​
甚至打開人工CPR機器，不斷按壓、急救到肋骨斷裂，​
直到真正死亡的案例。​
​
因為深愛不願放手，而讓親人臨別前遭受苦痛，​
正是許多家屬後悔的心聲。​
而這也是黃醫師想解開大家對於急救的迷思，​
讓病人能夠有尊嚴、舒適的離開。​
​
老實說，我連打耳洞都怕痛，​
我更不希望自己或親人這樣離開。​
這讓我思考了一件事，​
若我們能提前選擇，做好準備。​
你會願意找個機會和家人溝通一下嗎？​
​
二、DNR簽署仍需要你的家人同意支持和了解，​
並告知醫師。​
​
我知道他簽了ＤＮＲ，但就因為他是我的家人、​
我的親人，我無法見死不救。​
醫生請你救救他吧！如果你不救，我會．．．​
於是醫師只好啟動急救機制，​
直到壓到病人肋骨斷裂，口鼻冒出血來。​
直到急救無效，護理師會將現場處理乾淨，​
你不會知道家人死前經歷了什麼。​
​
換位思考，如果躺在病床上的是你，你會不會生氣？​
為何你們不尊重我的決定？為何要救我？我好痛。​
我好氣，生命是我的，為何我不能掌控我自己的選擇？​
​
書中提到了黃醫師的恩師生前已簽下同意書，​
但在病榻前兒子仍要求急救，最後救回。​
黃醫師看著老師嘴巴插管說不出話，眼眶泛著淚，​
卻無法幫助老師的為難傷心心情，​
是一個看盡生死的醫師最無助的一刻。​
​
我閱讀後，思考著如果想簽下DNR同意書，​
必須花很大的力氣和家人溝通，​
並請他們理解支持和同意，簽名送件很容易，​
而這件事才是最大最耗時也必須的挑戰。​
​
三、簽署了，不代表醫生會見死不救，​
除非死亡已不可避免。​
​
許多人擔心簽下同意書，​
萬一發生意外，醫生看到註記會不會見死不救，​
​
黃醫師在書中也提到只有在死亡不可避免的病況下，​
醫師才會告知家屬這件事。​
​
我認為這是最不需要的擔心，生死有命，​
能不能救得回，交由專業判斷。​
每個人對醫師來說都是一條寶貴的生命，​
這也是每一位醫師學醫的使命。​
​
看完書之後，我更關心的是家人的感受。​
不管親人是否善終，喪親終究還是一道創傷。​
因為決定要不要施行急救這回事，​
對家屬來說是一個沉重的壓力。形同決定親人的生死。​
​
不管怎麼決定，都可能會後悔傷心，​
更重要的是，臨終前還有機會、還有說話的時間，​
值得我們把握，去說些什麼，​
好好表達愛意、道歉、好好說再見。​
​
兩年前，送走我的寵物兔Maru時，​
我曾猶豫要不要送他去急救，​
當時他已經在休克狀態，​
非常的喘，全身癱軟無力。​
後來，我選擇陪他到過程結束。​
​
我們全家一起在客廳陪他，​
哭著要他放輕鬆，告訴他我們都愛他，​
也謝謝他的陪伴。​
​
儘管如此，我有時還是會後悔覺得當初應該拚急救，​
也許還能救回。​
但跟他道別的那刻，我永遠也不會忘記我有多愛他，​
已經深深刻在我心裡。​
​
面對悲傷，流淚釋放是允許的。​
我覺得最重要的是，能好好和對方道歉、道愛和道別。​
這個過程，帶給悲傷的人釋放的力量。​
​
我的奶奶是在家裡離開的，​
還好奶奶沒有受到急救的折磨。​
但我很遺憾奶奶走的時候，​
沒能看到她最後一面，她走的前兩天，​
我去她房裡看她，但我沒有去摸摸她，​
後來就再也沒機會了。​
​
所以，當還有時間去實踐這件事，​
把愛和感受傳達給家人，才不會留下遺憾。​
​
對不起、謝謝你、我愛你，幾個簡單的句子，​
可以給彼此好大的愛的能量波。

想問問齋友們，你們的父母曾對你們說如果發生了甚麼意外時，他們選擇「放棄急救」嗎?

這幾天，有個著名的急診醫師白永嘉，在臉書上表示96歲的父親因病入院，他已經簽署了拒絕心肺復甦術同意書 ( Do not resuscitate，縮寫DNR），並且說明這決定是因為深愛家人。

我記得大概十多歲，父母就對我告誡，萬一哪天他們發生意外，半死不活，千萬不要嘗試急救。我記得從我懂事以來，父母就非常害怕插管。而這些觀念也深深影響著我。

不過這幾年認真想想，能「放棄急救」的家庭多半都較為幸福。

1.首先是生命觀念。若家庭的價值觀仍屬老舊，願意放手讓家人安詳離去的意願就低，更別提還會有那種親戚之間會用「放棄急救」來撻伐簽署文件的鳥事。

2.遺產問題。若長輩身上仍有大量資產未妥善安排，而正好又有多名子女的情況下，彼此恐怕吵遺產都來不及，誰去簽了DNR大概就會被罵一輩子，甚至還會懷疑與長輩有暗盤交易。

雖然父母長年來多次囑咐我這些事情。但我至今日也不敢說，真到了那一天，我會毅然決然的下此決定。這背後要承擔的責任太重，需要擁有的親情太深。我認為能為長輩勇敢簽署DNR的人都值得欽佩。

我真心希望自己未來也是那個勇敢的人。
無論是有勇氣簽署，或者告訴別人自己DNR的意願。
