【藥事知多D】一物兩用：Amantadine

一種藥往往可以有很多種不同的用途。

不過這些用途偶爾可能會風馬牛不相及。

例如Amantadine。

那Amantadine到底有什麼用途呢？

至於常用的用途主要一般有以下兩個：

一、帕金遜症(Parkinson's Disease)

雖然暫時還是不太清楚具體的作用原理，不過一般相信Amantadine能夠抑制NMDA受體(NMDA Receptors)，主要適用於紓緩一些帕金遜症的症狀，例如運動徐緩(Bradykinesia)、僵硬(Rigidity)、震顫(Tremor)。

二、甲型流感病毒(Influenza A)

在藥理上，Amantadine主要透過阻斷甲型流感病毒的M2離子通道(M2 Ion Channel)和抑制病毒脫殼(Viral Uncoating)，達到抗病毒的效果。

不過Amantadine較容易會出現抗藥性(Drug Resistance)。

實際上，只要一丁點兒的基因突變，例如點突變(Point Mutation)，即僅僅改寫一個核苷酸(Nucleotide)，便已經足以誘發交叉抗藥性(Cross-resistance)。[1]

在美國，隨著抗藥性愈來愈嚴重，現在Amantadine一般不再建議用來治療、預防流感。[1]

當然同一種藥，用途或許不同，不過副作用總會相同吧？

在副作用上，主要是頭暈、口乾、幻覺(Hallucination)，偶爾還可能會誘發網狀青斑(Livedo Reticularis)。

看！Amantadine既是帕金遜藥，又是抗流感藥。這兩種用途是否有點牛頭不對馬嘴呢？
